Y9 & Y10 School Leaver Booster jabs, Monday 26 September 2016

For Parents and Carers of Years 9 and 10 Students

Following the recent Government directive, the vaccination programme for the School Leaver Boosters (SLB) has been transferred from the GPs to the Schools. These boosters are the continuation of the tetanus, diphtheria, polio and meningitis C protection programme begun as infants.

A letter containing details about this year’s SLB vaccination programme can be accessed here. You should also download the consent form which you will need to complete (indicating whether consent is given or not), sign, date and return to the Academy by Wednesday 21st September 2016.

The SLB jab programme has been scheduled to take place on Monday 26th September 2016.

Please would you ensure, as far as possible, that your child attends school promptly on the day of the vaccination and that, more than any other day, they are strongly encouraged to eat breakfast before coming in to the Academy. In addition, please could your child come in to school on the day wearing their PE shirt or short-sleeved school shirt, bringing their full school uniform into which they are to change after their jabs.

Please do not hesitate to contact me by phone on 01243 546828 or by email at stn@ormistonsixvillages.ork.uk should you have any queries about the SLB Vaccination Programme.

This communication has been sent by email via the Connect school/home communications system. If you have not received the email, please complete and return this details form to arrange for you to receive future Connect email communications.

Our Communities
